FNP Position in Northern VT | Home-Based Care

Optimum has partnered with a leading client seeking full-time Family Nurse Practitioners to join their teams in scenic northwestern Vermont.

This position will be providing home-based palliative care for patients in their homes, and the ideal candidate will have family practice or geriatric experience.

Position Details:

• Open to ANP, FNP, & GNP
• Schedule: Day shifts, 8a-5p with no call
• Daily volume: 3-4 patients
• EMR: Welkin
• Competitive compensation
• 2+ years of experience preferred
• 45-minutes west of Stowe and Montpelier
