FNP Position in Southern NH | Home Health Care

Optimum has partnered with a client in southeastern New Hampshire, 30-minutes outside of Boston, Massachusetts, seeking a Family Nurse Practitioner to join their team.

This position will be providing home health care, being a support system for patients to prevent unnecessary or avoidable hospital and ED visits.

About the Position:

• Schedule: Home health visits, Monday-Friday, 8a-5p
• No call responsibilities
• Daily Census: 12 patients
• EMR: Welkin
• Competitive salary
• Must be board-certified
