Optimum has partnered with a leading client seeking full-time Family Nurse Practitioners to join their team throughout New York. These locations include downtown New York City, north-central, and southeastern New York. This position will be providing home-based palliative care for patients in their homes, and the ideal candidate will have family practice or geriatric experience.

Position Details:

• Open to ANP, FNP, & GNP
• Shift information: Day shifts, 8a-5p
• No call shifts are required
• Daily volume: 3-4 patients
• EMR: Welkin
• Competitive compensation
