Opportuity Details:
- Schedule: Monday-Friday, 8a-5p
- Daily Volume: 10 admissions
- Patient Population: Geriatric
- Duties: Managing chronic conditions, preventative care & assessments, medication management, and care coordination
- Support Staff: MA and RN
- EMR: Cerner
- Must be board-certified
- Paid travel and expenses
